Re: xxxxx xxxxx

Ms. Gibbs & Ms. Perry:

I, Fred Nance Jr., have Power of Attorney for Health Care over xxxxx xxxxx. On November 18, 2009 I faxed copies of my Power of Attorney to Ms. Perry. In December of 2009 I talked to Ms. Gibbs requesting xxxxx xxxxx’ medical records. I have not received the medical records for xxxxx xxxxx.

On January 21, 2010 I called Sacred Heart Home. I talked to Ms. Regina Rawls of Medical Records. Ms. Rawls informed me that she was still working on getting xxxxx xxxxx’ medical records together for me. Ms. Rawls stated that she was very busy, and has not had time to get xxxxx xxxxx’ medical records together. I reminded Ms. Rawls that she has been getting xxxxx xxxxx’ medical records together since November of 2009.

On January 21, 2010 I visited xxxxx xxxxx at Westwood Manor Nursing Home. I was informed that Sacred Heart Home forwarded $30.00 to Westwood Manor stating that it was Ms. xxxxx’ final balance. On January 21, 2010 I called Sacred Heart Home. I talked to Victor Arnette, who said he was the Business Office Manager. I informed Mr. Arnette that I wanted a complete accounting of xxxxx xxxxx’ income and expenses for a significant period of time. I informed Mr. Arnette that I had Power of Attorney for Health Care for xxxxx xxxxx. Mr. Arnette informed me that I needed to provide him with a “certified” copy of my Power of Attorney.

On January 21, 2010 I called Sacred Heart. I talked to Ms. Arnetta Perry. I explained to Ms. Perry that I had been talking to her staff and being place on hold for at least 45 minutes, as I have been transferred from person to person, and department to department. Ms. Perry promised to facilitate my request for xxxxx xxxxx’ medical records, provide a complete financial account of monies received and spent, and her personal property. I informed Ms. Perry that xxxxx xxxxx told me she had a television that was never given back to her when she was transferred to a hospital for treatment in the past.

I am attaching a copy of the Power of Attorney for Health Care with this letter. This letter will be sent by certified mail. I will fax these documents also. Ms. Perry informed me that she would call me at xxx-xxx-xxxx when she has completed gathering the information requested in this letter. I informed her I would personally come and pick up this request.
